  • Full extension is required — partial flexion closes the joint.

Image evaluation

  • Open joint space
  • Lateral flexed 90° with 3 concentric arcs

Common mistakes

  • Partial flexion on AP → closed joint
  • Rotated lateral

Trauma modifications

  • Acute injury: do 2 APs (humerus parallel, then forearm parallel) instead of forcing extension.
  • Coyle (axial oblique) for radial head/coronoid in trauma.
