Anatomy coverage

C1 (or skull base) through C7/T1 junction.

Tech pearls

  • Pull shoulders down with weights/straps for C7 visualization.
  • Open mouth: line from lower edge of upper incisors to mastoid tip perpendicular to IR.

Image evaluation

  • Lateral: all 7 cervical vertebrae and C7/T1 junction visualized
  • Open mouth: entire dens between lateral masses of C1
  • AP axial: open IV disc spaces, no mandible over C-spine

Common mistakes

  • Missing C7 on lateral
  • Mandible over C1/C2 on open mouth
  • Forgetting 15° angle for AP axial

Trauma modifications

  • See dedicated 'Trauma C-Spine' entry — cross-table lateral first, do not move patient until cleared.
