Needle entry at L3–L4 or L4–L5 interspace, midline.

Central ray

Lateral horizontal beam to needle hub for guidance.

IR placement

Fluoro field.

Tech pearls

Always count vertebrae from L5–S1 to confirm level.
Use lowest pulsed fluoro rate possible.
